To obtain a copy of your police report, you must make your request in writing by filling out an application and submitting it to the Fortuna Police Records Department along with the proper fees.

You may submit your application in person or by mail to:

The fee is $9 for a report.

For questions please call (707) 725-1424 between the hours of 8:00 am and 5:00 pm PST.

You will be notified when your report is ready to be picked up. You must show your government issued ID when you pick up your report.

If you would like us to mail your report to you, it will be mailed to the address you gave the officer at the time of the report.
